Note:

TEAM enrollment must be completed by the TEAM CAPTAIN; only register your TEAM once. Only players listed on your roster and that have a PLAYER contract form on file will be permitted to participate on your team.

BEFORE PROCEEDING YOU WILL NEED THE FOLLOWING INFORMATION:
1) Team Name (Keep names family-friendly and fun; names will be confirmed by Town when Team packet is sent)
2) Team Captain/Representative
3) Team shirt color (Jerseys are not provided by the Town, team color will be confirmed when team packet is sent)

A separate registration packet will be emailed to the team captain/representative within three business days and must be filled out prior to the registration deadline, Wednesday, April 8th. This packet will include the rules, regulations, player contracts, and roster sheet to be filled out with all of your teams' participants. This packet may also be picked up in person at Farmville Town Hall.

Individual players must also complete a separate individual player contract form prior to the deadline. Completed player forms should be turned in with the Team packet by the team captain; individual player forms will not be accepted. Teams in violation of policies are subject to disqualification.
